🧾 Insurance & Hospital Bills for Foreign Visitors
Many hospitals want payment first. International departments cost more but reduce language friction. Keep every receipt for insurance claims.
Call your insurer before expensive care when you can safely wait.
Scope: national
TL;DR: Know whether your policy is direct bill or pay-and-claim. Bring passport + policy number + emergency assist phone. Photograph every bill and diagnosis note.
Prerequisites:
- Travel/expat medical policy PDF offline
- 24/7 assistance number saved as contact
- Payment capacity: card + Alipay + cash buffer (Money Runtime)
- Hospital access for how to enter care
Problem
Visitors assume “insurance card = walk out free.” In China, self-pay then claim is common outside a few international clinics. Surprise bills and missing paperwork kill reimbursements.
📋 The Runbook
1. Before you need care (pre-trip)
- Confirm coverage includes mainland China and your activity type.
- Note: deductible, max, COVID/sports exclusions, outpatient vs inpatient.
- Save policy number, member ID, assist phone, email for claims.
- If you have chronic meds, check Prescription refill + medication legality.
2. Choose facility type
| Facility | Pros | Cons |
|---|---|---|
| Public ER (三甲) | Everywhere; 120 routes here | Crowds; language; process heavy |
| International dept / clinic | English more common; itemized bills | Higher cash prices |
| Insurer network hospital | Possible direct billing | Must call assist to confirm today |
Action: For non-ambulance cases, call insurer assist first if time allows — ask “cashless / direct bill available at [hospital]?”
3. On arrival at hospital
- Passport + payment method ready.
- Say you have international insurance; show card/PDF — do not assume automatic coverage.
- Ask for itemized invoice / 明细 and diagnosis papers.
- For work claims, request fapiao where possible (Money Runtime).
4. Documents to capture (minimum claim pack)
- Passport bio page
- Policy number screenshot
- Admission / outpatient record
- Diagnosis (诊断)
- Itemized bill (费用明细)
- Payment proof (card slip / Alipay record)
- Fapiao if issued
- Prescription list
Photograph paper forms before they leave your hands.
5. After payment
- Notify insurer within policy window (often 24–48h for major cases).
- Upload claim via app/email as required.
- Keep originals until claim closes.
- Verification: Claim reference number issued.
🚨 Fallback (Plan B)
| Situation | Action |
|---|---|
| Hospital demands full prepay | Pay to treat first if emergency; claim later — life > paperwork |
| No English | Translation app + phrase tools; companion if available |
| Card declines large bill | Second card; wire options via insurer assist; embassy list is last resort not ATM |
| Insurer denies cashless | Self-pay path; insist on complete paperwork |
| Ambulance 120 | Goes to nearest appropriate public hospital — not always your preferred international clinic |
| Dispute amount | Written itemization; insurer medical team; do not sign blank forms |
Cost shock control (order-of-magnitude mindset)
Prices vary wildly by city and facility. Mental model only:
- Simple outpatient consult + basic tests: often manageable on a card but not “EU public health free.”
- ER imaging + night stay: can jump to amounts that max a travel card — call issuer / insurer early.
- Always ask estimate (大概多少钱) before non-urgent add-on tests.
This is not a price list — verify on site.
💡 TechDad’s Tips
- Put assist number in phone as ICE-INSURANCE.
- International clinics often want passport + credit card imprint even with insurance letters.
- Kids: carry allergy card (generator tool) into ER.
- If flying soon after care, ask doctor for a fit-to-fly note when relevant.
